How to Add Instructor Questions to Student Feedback
2-Step Process
Step 1 – Add Questions to Instructor Question Bank:
Within 3 days of receiving the email with subject line "Student Feedback: Check Instructor, Timing, Add Questions", reply to this email and make sure [email protected] and [email protected] are included. The questions will be reviewed by the office.
Questions posed should be useful to you in your teaching of this course, and students should be able to answer. They can be formative or developmental. Department chairs and other school-based administrators will not be able to see the results to instructor-added questions. Questions can be open-ended (comments), scale with average, ordered list without average, single answer without any order, or check-all that apply. All instructor-added questions are optional. Avoid questions that can be used to identify any individual student. For each question submitted, please specify type of question, the question text (what the student sees), short question text (displayed in the results table), answer text (if applicable), and related value (if applicable).

Step 2 - Apply Instructor-added Questions to Course:
Please wait 3 business days for newly submitted questions to be added to the question bank. Previously added questions can be added immediately. Instructors can add up to 3 questions to each course. You may choose questions that have been submitted by other instructors. There are about 90 questions in the Instructor Question Bank as of the beginning of June 2024. Please apply questions to your courses 3 days before the first day of each course's feedback period. After that, the system will no longer allow changes so that the questionnaire can be finalized before it is released to students.

Under "Surveys Waiting to Be Opened" there should be course(s) that have not yet opened for feedback. Click on the plus sign or "Add Questions" for the course you would like to add questions. You will have choices "All Classes With This Course Title", "This class only" (specific to this semester), or "Every Class You Teach". After selecting your choice, you will be directed to the Instructor Question Bank. Questions are displayed by type (single choice, multiple choice - no average, text questions). All of these questions have been submitted by fellow instructors. Some are broad that can be used in any course, and some are very specific that might only be used by the instructor who submitted the question. (There is a label that says create new question, that is an incorrect label in the system and not available.) After selecting your question(s), click "Add selected questions" at the bottom of the page.
